Integral Health does not guarantee that Website submissions, portal messages, emails, text messages, appointment requests, or other electronic communications will be reviewed immediately.
4. ELIGIBILITY AND LOCATION
You must provide accurate information regarding your identity, age, current physical location, medical history, and other information reasonably necessary to determine whether Integral Health can provide services to you.
Healthcare services may only be provided when legally permitted and when an Integral Health provider is appropriately licensed or otherwise authorized to provide care in the jurisdiction where the patient is physically located at the time care is provided.
Integral Health may decline, discontinue, or limit services when required by law, professional standards, clinical judgment, provider availability, licensure restrictions, or patient safety considerations.
5. APPOINTMENTS AND ONLINE SCHEDULING
The Website may allow users to request or schedule appointments.
Submission of an appointment request does not necessarily guarantee an appointment or establish a provider-patient relationship.
You are responsible for providing accurate contact and scheduling information and for attending appointments at the scheduled time.
Cancellation, rescheduling, late-arrival, no-show, deposit, and other appointment policies may apply and may be provided separately during scheduling or registration.
Integral Health reserves the right to reschedule or cancel appointments when reasonably necessary.
6. TELEHEALTH SERVICES
Integral Health may offer telehealth services when clinically appropriate and legally permitted.
Telehealth has inherent limitations and may not be appropriate for every medical concern. A provider may determine that an in-person examination, laboratory testing, diagnostic imaging, specialist evaluation, emergency evaluation, or other form of care is necessary.
Use of telehealth services may require completion of additional consent forms and acknowledgment of telehealth-specific policies.
Patients receiving telehealth services must accurately disclose their physical location at the time of the encounter.
7. MEDICATIONS AND PRESCRIPTIONS
Scheduling an appointment, completing an intake form, paying a consultation fee, requesting a medication, or previously receiving a medication does not guarantee that a prescription will be issued or renewed.
All prescribing decisions are made by the treating healthcare professional based on independent clinical judgment, medical necessity, applicable laws, professional standards, contraindications, risks, benefits, available medical information, and other clinically relevant considerations.
A provider may require laboratory testing, medical records, follow-up evaluation, physical examination, monitoring, or other information before prescribing or continuing a medication.
Integral Health does not guarantee that any specific medication, compounded medication, hormone therapy, weight-management medication, peptide-related therapy, supplement, or other treatment will be appropriate or available for any particular patient.
8. COMPOUNDED MEDICATIONS
When clinically appropriate and legally permitted, an Integral Health provider may prescribe a compounded medication.
Compounded medications are prepared by compounding pharmacies and are not FDA-approved in the same manner as commercially manufactured FDA-approved drug products. The FDA does not review compounded medications for safety, effectiveness, or manufacturing quality before they are marketed in the same manner that it reviews FDA-approved medications.
When applicable, patients will receive additional information and consent documentation regarding compounded medications, their potential risks and benefits, available alternatives, and the pharmacy dispensing the medication.
Availability of compounded medications may change due to federal or state law, regulatory action, pharmacy availability, medication shortages, or other circumstances outside Integral Health’s control.
Integral Health does not manufacture medications and does not control third-party pharmacy production, shipping, inventory, or dispensing operations.
9. LABORATORY TESTING
Certain services may require or recommend laboratory testing.
Laboratory results must be interpreted in the context of the patient's medical history, symptoms, examination, medications, and other clinical information. Reference ranges alone do not determine whether treatment is necessary or appropriate.
Patients are responsible for completing recommended laboratory testing and follow-up monitoring as directed.
Integral Health may decline to initiate or continue certain treatments when clinically necessary monitoring has not been completed.
10. INDIVIDUAL RESULTS
Healthcare outcomes vary significantly among individuals.
Integral Health does not guarantee specific outcomes from any treatment or service, including:
Weight loss or body-composition changes;
Hormone levels or symptom improvement;
Increased energy, libido, or performance;
Improvement in metabolic markers;
Hair or skin changes;

11. PATIENT RESPONSIBILITIES

Integral Health may coordinate care with independent pharmacies, laboratories, payment processors, scheduling platforms, technology providers, shipping carriers, and other third parties.
These organizations are separate entities and may maintain their own terms, privacy practices, fees, processing times, and policies.
Integral Health is not responsible for circumstances outside its reasonable control involving an independent third party, including shipping delays, carrier disruptions, pharmacy processing delays, medication inventory, laboratory processing times, or third-party technology outages.
Nothing in this section limits any rights or remedies that cannot lawfully be waived.
14. SHIPPING AND DELIVERY
When medications, supplements, or other products are shipped by a pharmacy or other third party, estimated shipping or delivery times are not guaranteed.
Patients are responsible for providing an accurate shipping address and following storage instructions supplied by the dispensing pharmacy, manufacturer, or healthcare provider.
Questions concerning a medication that arrives damaged, delayed, unusually warm or cold, or otherwise potentially compromised should be reported promptly. Patients should not independently assume that a potentially compromised medication is safe or unsafe to use without appropriate guidance.
15. ELECTRONIC COMMUNICATIONS
By providing contact information, you may consent, as permitted by applicable law and your communication preferences, to receive communications related to appointments, treatment, billing, administrative matters, and other services.

Electronic communications have inherent privacy and security limitations. Sensitive medical information should be communicated through approved secure methods whenever available.
